What the medical statement is for

The medical statement is for our verifiers who consider and make decisions for School High Health Needs Fund (SHHNF) applications.

It should support the application by providing relevant information which will enable the verifiers to gain an understanding of the current health needs of ākonga | students.

Medical statements should be completed by a medical specialist. In most cases this is a paediatrician or GP. However we can also accept statements completed by a

  • clinical nurse specialist
  • diabetes nurse specialist
  • nurse practitioner.

Medical statements should be dated within 6 months and should not provide generic information about a health condition.

The medical statement needs to provide specific details about the individual ākonga including:

  • health condition/s, date of diagnosis (if applicable) and the effects currently impacting the ākonga at school
  • current medications or treatments including any upcoming changes and if required to be administered at school
  • severity and nature of the health risks for the ākonga.

Type 1 diabetes

Medical information required for the application

  • Confirmation and date of Type 1 Diabetes diagnosis.
  • Type of insulin delivery (that is, injection or pump).
  • Any other relevant health or developmental issues.

Medical information required for the review application

  • Confirmation and date of Type 1 Diabetes diagnosis.
  • Any changes to the method of insulin delivery within the past 12 months.
  • Any changes to the method of insulin delivery scheduled in the next 12 months.
  • Other relevant health or development issues.
