Leaving school

You must contact your local assistive technology coordinator (ATC) at your regional office if a student with assistive technology is leaving school or no longer needs the technology.

Regional offices

Your school must liaise with the student's whānau | family and the ATC. The ATC will put the information together to send to the regional technology coordinator (RTC). They will decide what happens with the technology and discuss this with your school, the student and their family.

Once you know what is happening with the assistive technology, you can fill in the transfer form.

Changing school

If the student is transferring to another school, both the current and the new school must fill in the transfer form. This transfers responsibility for the assistive technology to the new school. The responsibilities for each school are set out in the transfer form.

If the assistive technology is not needed at the new school, it should be returned to the ATC at your regional office.

Assistive technology transfer form

Complete this form and send it to your ATC when a student is changing schools, leaving school, or no longer requires the assistive technology.

If the technology is no longer needed by the student return it to your regional office with a completed first page of the transfer form.

If assistive technology items are lost or damaged before they are transferred to a new school, the old school is responsible for replacing them.
