Contact us to start planning how to support your student's needs
When you receive confirmation that an ākonga | student will receive the School High Health Needs Fund, you should contact your regional office. A service manager will ensure you have a key worker from our team to support you.
Your key worker will help you to develop an individual care plan (ICP). Once you have the ICP, you will be able to employ a teacher aide or support person to support your student.
What is an individual care plan
The individual care plan (ICP) will have:
- a brief summary of student health needs
- the care and supervision tasks the support person completes, how long each task takes and how many times a day and week the tasks are completed
- strategies or measures to keep both adult and child safe
- what tasks students can do independently and what tasks they need help with
- plans and strategies to help the students towards managing their own care needs to the best of their ability and as quickly as possible
- how the school will monitor the care plan during the year to make sure it stays at the right level for student needs
- how the school will evaluate the care plan to determine the ongoing level of need for care and supervision in relation to how the student is gaining independence
- signatures of mātua | parents or caregivers and the educator.
Review the ICP at least once a year
If the needs of the student change you will need to review the plan more often. When you review an ICP you should hold a meeting that includes:
- parents and guardians
- educators (including your key contact), and
- a health care professional (if appropriate).
Employing a support person
You will need to employ a suitable support person or teacher aide for your student.
As the employer, the terms and conditions of employment of the support person are the responsibility of the school.
You must determine:
- the appropriate grade, and
- hourly rate of pay.
