You must work alongside mātua | parents or caregivers of the ākonga | student and the regional office to complete the form.

Applications to extend the School High Health Needs Fund (SHHNF) should be made about 1 term before the fund ends. We send a reminder letter to whānau | families of the student, their school and their regional office around this time.

Letters will not be sent for students who are funded for a pre-determined short period of time.

What you need to apply

To apply to extend the SHHNF, you will need to provide at least 10 weeks of daily records of a student's health support at school. We will use this to review the student's progress.

Daily records should include:

  • the frequency and nature of the health concern while at school
  • any times the support person had to intervene to stop an incident from happening
  • details of the support required following an incident.

You will also need:

  • a current care plan
  • a health care plan, where relevant.

If the student has type 1 diabetes, complete this application form.

What happens after you apply

Once you have sent the completed form to us, you will get an automated reply. This is followed by a formal acknowledgement of your application for a review of the SHHNF from us within 5 working days.

Our review to extend funding normally takes 30 working days but can be longer if we need more information from you.

If the application for extension is approved

Parents and schools are told of the outcome of their applications on Mondays. The school's enrolment system (ENROL) is updated with this information. 

To check if SHHNF is in place for a student, view their details in ENROL.

If the application for extension is declined

You will receive a letter explaining why the application was declined and your next steps. 

You can request a review of our decision. You will usually need to provide more information supporting the application. Our verifiers will review your request and the new information you provide.
